SERVICE REQUEST

New York City's 311 Citizen Service Center provides easy access to all non-emergency City services. Dial 3-1-1 (local call) to reach a Call Center representative who will answer questions about City services or register a compalint. Tracking numbers are provided so that callers can follow up on the status of their requests. more info

CB4's District Office will also report problems or convey service requests to City agencies. To register a complaint or service request with CB4, fill out the form below.
